New patients are often required to obtain a physician referral prior to your visit. That doesn’t mean you’re on your own, the team at CSPM is here to help in a number of ways. You can begin by filling out the form below or printing the patient referral form (click here to access the physician referral form) to give to your primary care or family medicine doctor.

You can also refer your family physician to our educational videos on chronic pain treatments. These short videos will allow you and your primary care doctor to decide on the best course of action. Our educational videos and resources can be found here.

As a new patient, you’ll have an initial screening with a board-certified Pain Management physician. Once we know the source of your pain and any other information you think important we can help you begin your path to living pain free.

Our team of Doctors, Nurses, and Coordinators have the experience to help you wherever you are on your journey to relief. If for some reason CSPM cannot help you, we will do our best to find someone who can. We specialize in caring for patients with Chronic Pain (Long-term pain that can last months, even years), Workers Compensation, Migraine Relief, and Personal Injury.

CSPM and its Board-Certified Interventional Pain Medicine (often referred to as Pain Management) Physicians share a common goal, to use treatment techniques to relieve your pain and perform appropriate procedures to minimize the use of prescription drugs.
